Immunization Requirements

Minnesota law requires children enrolled in school to be immunized against certain diseases or file a notarized legal, medical or conscientious exemption.

Please be sure your child's immunizations are up to date prior to the start of the school year.

Immunization Laws for Schools, Child Care and Early Childhood Programs 


Immunization Requirements to enter Kindergarten

    • 3 Hepatitis B
    • 5 Dtap (diptheria, tetanus, pertussis)
    • 4 Polio
    • 2 MMR (measles, mumps, rubella)
    • 2 Varicella (chickenpox)


Immunization Requirements to enter 7th grade

  • 3 Hepatitis B
  • at least 3 Dtap (diptheria, tetanus, pertussis),
  • plus one more Tdap at age 11-12 (Booster shot) 2014 this replaces the Td immunization requirement.
  • at least 3 Polio
  • 2 MMR (measles, mumps, rubella)
  • Meningococcal, new 2014 for students entering the seventh grade
  • 2 Varicella (chickenpox)
